The little black dot marking the signal's location is simply to help the conductor know which signal he/she is looking at. Signals can move from time to time as MoW works on track and moves signals and control points around. There is no circling of the dot. there is only one dot per box. if you pass that specific signal, just filling in the box with the corresponding aspect abbreviation is sufficient.

These current signal awareness forms were designed by local crews and management together in an effort to make them more user friendly and understandable.

If a crew dies (times out of service) and hasn't completed a trip through a subdivision, does the new crew resume using the same form the prior crew used or do they start a new one?

On the UP, if the crew dies on the hours of service, the old condr. takes his form with him, and the new condr. starts one of his own.

Below, a scan of a complete BNSF Railway WAT-BAR Signal Awareness Form. Any mainline train heading east out of the Ports of LA/LB or Watson Yard will have one of these on board to fill out. As you can see, there are a lot of signals (120+) between the ports and Barstow.
